Failures are a common phenomena in civilisation. Things fail and society responds, often very slowly, sometimes inappropriately. What kinds of things go wrong? Why do they go wrong? How do people and organisations react to failures, and what are the best ways to react? William B. Rouse takes an analytic approach to these questions and addresses eighteen well-known cases of high-consequence failures. He employs a multi-level framework to integrate findings across the case studies, and in turn uses these to outline a conceptual approach to integrated failure management. Though diverse in their causes and outcomes, his analysis shows that the conceptual design of an integrated approach to failure management can encompass each of the case studies, all of which would have benefitted from the same conceptual decision support architecture.

"The first members of Annelida were formally named by Linnaeus (1758) and in the intervening period there have been thousands of others added. We estimate that the current number of accepted species level taxa is just under 21,000, though several thousand more have been named and are considered invalid. This is based on a count of the estimates presented in the taxonomic chapters of this volume. Over 8,000 of these are members of Clitellata with the remaining ~12,000 belonging to the other traditional members of Annelida, plus more recent additions such as Sipuncula and Thalassematidae (= Echiura). The 77 taxonomic chapters that comprise the bulk of this volume are organized largely in accordance with a synthesis of the state of knowledge in early 2021 and shown in Figure 1.1 and simplified somewhat in Table 1. We have applied more inclusive names for clades incorporating names in common usage, or resurrected others for reasons that are outlined below. Table 1 does show a system of Linnean taxonomic ranks applied to the major clade names. We provide this as a coherent system for those who wish to adopt such ranks, and we understand that they are currently required in some databasing systems. We hope though that on using this volume and seeing that the lack of taxonomic ranks, it will be apparent that they add little utility and are best ignored"--
